Helen (Winn), die afkomstig is uit een fatsoenlijk milieu, belandt vanwege een illegale abortus in het ziekenhuis. Na haar ontslag uit de kliniek komt ze geheel ontredderd terecht in de armen van de verslaafde kruimeldief Bobby (Pacino). Aanvankelijk is ze nieuwsgierig, maar langzamerhand raakt zij ook in de duivelsachtige cirkel van de verslaving verstrikt. Ze moet zich prostitueren met Hank (Bright), het inbreker-broertje van Bobby om aan geld voor stuff te komen. 'Needle Park' is de bijnaam van de kruising in West Manhattan, waar de heroïne verhandeld wordt en waar tijdens een bust Bobby gepakt wordt, en in de bak komt. Als Helen hem afhaalt nadat hij gezeten heeft, gaat het verder bergafwaarts met hen. Een uitstekende film over het drugsprobleem van toen met goede hoofdrollen van een regisseur die tot de nieuwe generatie van veelbelovende filmers gerekend wordt.

Bobby (Al Pacino) is a heroin addict who lives in "Needle Park," the nickname for an area on the Upper West Side of Manhattan where junkies congregate. He meets Helen (Kitty Winn), a lonely homeless girl, and they fall in love. However, Bobby also introduces Helen to heroin, and she eventually becomes addicted, too. As Bobby and Helen become more and more dependent on each other and on heroin, their need for money to feed their habit grows, resulting in crime, desperation and betrayal.

In New York City, soft-spoken Helen returns to the apartment she shares with her boyfriend Marco after enduring an unhygienic and inept abortion. Although Ellen becomes ill from the procedure, Marco is satisfied because he was able to procure the abortion through bartering, and thus did not have to expend any money. When Marco leaves briefly to collect some cash, Bobby, an amiable small-time drug dealer to whom Marco owes money, shows unexpected gentleness and concern for Helen. Soon after, when Helen is hospitalized for profuse bleeding, Bobby sneaks in after visiting hours and tells her that Marco has gone away. Briefly, Helen considers returning to her dysfunctional family in Fort Wayne, Indiana. However, after she is released from the hospital, she moves in with Bobby.
